We are seeking a dedicated and experienced Nurse Practitioner to join our team in providing exceptional care within our Residential Aged Care facilities. This role is pivotal in ensuring our residents receive expert clinical care, comprehensive assessments, and innovative management strategies tailored to their individual needs.
Key Responsibilities:
- Deliver advanced clinical nursing care and interventions, ensuring adherence to the Nurse Practitioner Standards of Practice.
- Manage complex health conditions, particularly for residents with advanced chronic diseases and end-of-life care needs, collaborating with healthcare teams to implement shared care plans.
- Conduct expert clinical assessments, develop treatment plans, and educate residents, families, and staff on palliative care and chronic disease management.
- Ensure clinical governance by adhering to compliance and quality standards, conducting regular audits, and developing clinical guidelines specific to aged care.
- Foster effective communication and collaboration with multidisciplinary teams, including GPs and allied health professionals, to enhance care outcomes.
- Participate in continuous professional development and contribute to mentoring and supervision within the nursing team.
We are looking for a Nurse Practitioner who possesses the following qualifications and characteristics:
- Registered Nurse with postgraduate qualifications and eligibility for endorsement with Australian Health Practitioners Regulation Authority (AHPRA).
- Minimum of 5 years of nursing experience, with at least 3 years in aged or community care settings.
- Extensive clinical knowledge in palliative and aged care, particularly in end-of-life and chronic disease management.
- Exceptional communication, negotiation, and leadership skills, with the ability to work autonomously and collaboratively.
